Document 4 — Certificate of Insurance
Certificate of Insurance (COI)
Upload your current COI from your insurance agent. We'll confirm coverage and track the renewal date.
Your certificate must show general liability coverage of at least $1,000,000 per occurrence / $2,000,000 general aggregate, with current workers' compensation coverage, and must name Lev Developments LLC, 5300 Doncaster Dr, Charlotte, NC 28211 as certificate holder and additional insured. Your insurance agent can issue this in minutes and will usually email it straight back to you as a PDF.

For your records
Lien waivers are part of doing business with any general contractor. A partial waiver goes with each progress payment, and a full waiver goes with your final payment on a job. Download both and keep them on file, so you are ready whenever a payment request calls for one.
